The mouth is divided into 4 quadrants, and local anesthetics are required to numb the tooth and gums for a painless deep-cleaning experience. Deep-cleaning removes plaque and tartar from below the gum line and ensures the root surfaces are smooth and cleaned. This allow the supportive tissues around the tooth to reattach to the root and tooth, thus reducing pocket depths and gums appearing pink and tight (which is how healthy gums should look).

The goal is to thoroughly remove plaque and calculus below the gum line with deep-cleaning (scaling and root planing). We also add to the mechanical cleansing (to remove the bacteria) using dental scalers, but we also use gum medicaments to ensure the root below the gum line gets the chemical cleansing to disinfect roots and teeth. Getting the gums back to health is the goal.

Periodontal scaling and root planing is not painful at all. However, there are sedation options available for you to request if you would like to make your visit as pleasant as possible with nitrous oxide gas, or oral sedation or IV deep sedation. Most patients can tolerate well with just a little patience and "tender-loving-care" techniques . Some patients will want sedation to be available to do with deep cleanings (requiring local anesthetics); in fact they usually want to to add a few more dental work with the same sedation visit. This will efficiently maximize the time under sedation. Multiple dental procedures done with sedation will cut the number of visits. Depending on the level of the sedation, the deeper the sedation, the more dental work can be added to the same sedation .

After scaling and root planing , you may feel mild discomfort to temperature and sometime pressure. Most patients report they took a couple of ibuprofen pills the first day. But you can certain take 2-3 tablets of 200 mg ibuprofen every 4-6 hours as needed for the first few days. This is a normal course of healing.
